ADMISSIONS PROCEDURE

The admissions process begins with a completed referral form. This form must be completed in its entirety by a physician, doctor, or licensed therapist who is familiar with the client’s case.

Once this form is completed either mail or fax it to:

Sanctuary Centers of Santa Barbara
Post Office Box 551
Santa Barbara, CA 93102
Attn: Clinical Director

Fax: 805.563.1977

After this form is received and reviewed the potential client or his/her family member will be contacted to arrange for a personal interview.

It is recommended that the prospective client and any family members who wish to accompany the client, plan to visit the program for a few hours.

For the Sanctuary House Program:

After the personal interview is completed Sanctuary Centers' Clinical Director will review the referral form and completed assessment with our Medical Director.

This process usually takes 5 working days.  At this point, Sanctuary Centers will contact the client and/or his/her family member to discuss the results of the assessment.

Should Sanctuary Centers and the potential client feel that Sanctuary House is a good fit the client will be placed on a waiting list for the next available opening.
